I'm pretty sure that's the Wilderness missions expansion box art, although it does look like the sort of thing that would be generated by the Region 1 Department of confused, macho and oversexed box art tragedies.

In hindsight, this might actually be one of the wallpaper extras GOG gives with the purchase, lol.

I found it in a game magazine PDF file of the German "PC Action". Since they actualy released it as pdf themselves (meaning, it's not a scan), all internal assets they didn't edit themselves are completely undamaged, and usually really hi-res. There were tons of nifty hi-res and beta images in there.
