It looks like Don Andy is using Blackbox. (bb4win)
I use it too (bbLean). It's so much better then explorer.exe

Stebbi seems to have some crazy theme installed

i use tuneup utilites it fixes your cpu, fixes the registry,cleans up useless crap, tells you everything about your pc and you can download themes as my one, deleta things beyond recovery, change boot screens and loads of other stuff.....

Exactly.

Have still some weird problems with that thing though, especially on startup, but I'm overall pleased with it.

Tried LiteStep first but it was somehow eating up my whole CPU with every click.

@efthimios:
It is, like Reup already mentioned, Blackbox for Windows, a shell replacement. It's easily skinnable, the Style I currently have is "System Shock 2004" with a background I found on the first site after googling for "system shock wallpaper"
Plugins installed: (in case you want to reconstruct my desktop, who knows )
BBAnalogExMod (the Clock)
BBDropFTP (the little FTP thingy at the top)
BBWinskin (so my Windows has the same style as Blackbox)

Oh, and in case you just want a nifty Windows-Theme, go patch your uxtheme.dll (google for it). You don't need programs running in the background and it's free (unlike StyleXP, WindowBlinds and stuff)

Fred Beltran, it's from a comic called Megalex. As for the suffering creature in her hand, it's by an unknown fan on the Giger homepage.
