As a kid (that was still in the times of socialism where you didn't have private dental practices, but you were assigned to a dentist) I got an elderly woman for a dentist. The problem was her hands. They were shaking all the time - so she had to hold the drill with both hands in order to steady it.
I don't know what happened when she was drilling my tooth, but she had to screw up big time!
I was 11 and she started drilling one of my teeth. Obviously her hand sliped or something with her drill still on, because she injured my gums - that's right, she drilled for a second or so in the gums and not the teeth. It took quite a while to heal, but it's OK now. The pain was great, but not unbearable. The biggest effect was a phychological one - I'm still mortally terrified of dentists and I get a high fiever just by knowing I'll have to go for a check-up.

My worst pain when I was 8:
First skating lesson at lesson at Ice Rink.
Everything was ok until the end.
Gone to return to thick wood barrier.
Tried to dodge somebody.
Done some sort of weird slide thing and hit barrier.
Hit head.
When woke up, bones cut through skin and in several pieces.
WORSE STILL TO COME.
Hospital said I was going to need pins in legs.
Bone specialist cleared up and put temporary cast on for weekend.
That night, cast splits open.
I fall out of bed in agony and cause even more damage.
Rushed to hospital. Healed in agonising 5 months.
Total breaks in Left Leg: 6
Total breaks in Right Leg: 5
