Getting three fillings sans anaesthetic springs to mind, but then I managed to recall a childhood experience.

I was about 7, and I on a skidoo with my cousin, age about 6, when we stalled in the middle of nowhere, a good 20 or so miles out of town with 5 feet of snow on the ground. We were stuck out there for about 5 hours before our folks tracked us down, by that time we were both frozen solid. Couldn't even move. We were numb at that point, but when we got back to his home, thawing out was one of the most physically painful experiences I've had to undergo. -48C weather and a stalled engine do not mix.

Another experience leaps to mind, a recent one, but I can't really relate it in full. Let's just say I was a nervous wreck for about 2 weeks afterwards, unable to sleep, or even close my eyes. I'm not sure if it could be classified as physical pain, it was my own cerebrium inflicting agony over my entire body. I still get flashbacks from that one, even thinking about it is making my heart race and my vision distort.
