I can't much speak for the behind-the-scenes sanitation at McDonalds outside of North America, but in the continental US it isn't exactly a beacon of high quality (though it's not quite so bad as it apparently is in the above mentioned locations). However, while it isn't the most upscale restaurant around, it is required to comply with health codes just as any other restaurant. While that isn't saying a whole lot, you'll generally hear the same horror stories pretty evenly distributed among all the fast food chains over here. Jack-in-the-Box with the tainted beef, Wendy's with the defacation in the food, etc. This like this just happen and will continue to because these places will hire just about anyone and sometimes "anyone" can mean less than upstanding members of society will be handling your food.
