To level up, I would stay in an easy act that gives quite a lot of xp, like Act 3 for example. You don't really want to put yourself in too much danger, because it's a total pain in the arse when you die if you're trying to level up.
By the way, I have a question to any good players out there - is level 21 a bit low to be fighting Mephisto with?
Because I panned Duriel (not least thanks to my equipment, which was a long sword with 3 Topaz gems in) at level 19, although he didn't drop anything

(although that's nothing to do with my level, and might even be normal, I suppose).
