It is very similar to Another World/Out of This World in the look, overall feel, movement, and the gameplay.

Very old game. You're this guy who has been in an accident or something, and is taken to the ICU in a hospital and is linked to an ECG, and while in this flat linning coma-like state he goes into a strange weird world.

Random things I remember:
1.) First stage, you're in a desert, with fish flying horizontally.
2.) Another stage there is a cute little girl with a ball, the ball later has deadly spikes and kills you.
3.) Each time you die, they show the scene in the hospital of you flatlinning.
