How high have you set the CPU cycles? Sound also requires some processing power, so higher cycles help a bit. Also click on Edit Configuration for DosBox and look for these two settings under [mixer]:
blocksize=2048 (default)
prebuffer=10 (default)
Change the default values for these two to 1024 each and see if that helps. Lower values values for blocksize, in some games, actually help for less stuttering sound (in games where it would still stutter under higher values) while higher values for prebuffer help reducing the frequency of those stutterings or eliminate them, hence the values I mentioned.
If the problems with the sound still persist, experiment with those two values. Also be sure to set the rate= under [mixer] to 22050 if it is higher than that - some games don't support Soundblaster 16, only older Soundblaster models, therefore no sample rates higher than 22050.
