Another World: lost the actual data, but it was a couple seconds above 15 minutes (lots of training)
Bloodrayne: 1hr 13min 6 sec to reach the final level (didn't code a Total Play Time mod back then, so I couldn't measure the actual completion time), with NO save/load and NO dying. My first try at this was nearly succesful... the game crashed after I've beaten one of the two final bosses.  Took me a couple of months to manage this again.

"Shackled" on ST - 12 seconds. Secret door to your right at the very start, go through it and the game ends. Super Lame. Didnt even know what I was doing, spent $30 on that game too!
