you need to press ctrl+f12 in order to speed up your dosbox. ctrl+f11 slows it back down, in case you went over the limits of your pc (then the game will be even more choppy). so just fiddle around with cycles and see what works best

OR simply change your core to auto and cycles to max, that will try to run dosbox on best speed possible

for the crappy sound effects- look for a file called setup or install or something like that and set sound blaster as your sound device
