yeah you do cut the opposite way with the scalpel *so its top right to bottom left* the only thing i can suggest is when you go through the other layers that you go as far to the corners as possible *otherwise you wont be able to get to the appendix as the hole will be in the wrong place*. But yeah the fourth layer you cut in the opposite direction which is the only difference from the layer above. Let me know how it works out for ya.
